This is a simple process and generally only takes about 10-15 minutes.  Because this form requires you to sign a statement certifying that you are qualified as an SAP, you cannot fill out the form online.  However, you may download our SAP Certification Form in Adobe PDF format, fill out the form and fax it back to us. 

To be added to our database:

1. Download IHSN's SAP Certification Form
This form requires Adobe Acrobat Reader (or any other PDF reader) which can be obtained for free by clicking here.   If you already have Adobe Acrobat Reader, then simply download the form by clicking here.

2. Fill out the SAP Certification Form
Fill out the entire SAP Certification Form, sign it and return it to IHSN along with a photocopy of your license or certification and verification of the DOT required qualification training and exam.
